Once you begin receiving services from Continuum, we partner with you and your family, your physician and other health care providers to maximize the benefit of the services we provide. As mentioned previously, we will work with you and your family to be clear about your goals. We will also communicate closely with your physician regarding your orders for treatment, responses, and any indications for change.
