Imagine a team of nurses, therapists, home health/hospice aides, medical social workers, chaplains and volunteers that are all focused on achieving your health care goals and also on ensuring that you are treated with respect and dignity and always have a sense of control over your own plan of care. The combination is quite empowering! Our nurses are expertly trained in symptom control and pain and disease management. Medical social workers provide education and coordinate community resources and  support. Certified nursing assistants provide help with activities of daily living, benefiting the patient and caregivers. Optional chaplain and volunteer services are available to hospice patients. Chaplains provide non-denominational, non-judgmental spiritual support. Volunteers provide socialization and helping hands to run errands or do any number of helpful tasks specific to individual needs. All services are under the direction of a registered nurse and physician with expertise in hospice care.

 

We understand that our patients are not the only ones touched by advancing illness. So, our care is not just focused on our patients, but also on those who love them. There are many ways we support the families of our patients. Family and caregivers have many questions when dealing with the illness of a loved one. We will provide resources and support services that help you and the family cope with the challenges ahead.
